New methods for treating varicose veins

Varicose veins affect those who work standing for long hours, such as traffic policemen. If varicose veins are not treated early, there can be complications such as chronic venous insufficiency.
Even though the disease is not life threatening, it can be debilitating if not treated properly. Many people do not seek treatment for varicose veins even when the disease is at an advanced stage as surgery is required in many cases, according to K.R. Suresh, Director, Jain Institute of Vascular Sciences (JIVAS), a unit of the Bhagwan Mahaveer Jain Hospital here.

Laser technique
However, for those who do not want to undergo surgery, various new treatments for both large and small varicose veins are now available, and patients can get back to work quickly after treatment.
One of these is "Endovenous Laser Ablation," a minimally invasive technique for treatment of varicose veins.
The treatment involves making a small incision in the affected part and the introduction of laser fibre. Laser is fired at regular intervals causing the varicose vein to collapse completely, Dr. Suresh says. "This is a convenient treatment for those who do not want to undergo surgery, and it is an outpatient procedure," he adds.
The Centre for Venous Disorders at JIVAS claims to be the first in the State and third in India to acquire the ELVes Laser System used for "Endovenous Laser Ablation." "Sequential Compression Therapy" is another treatment for severe swelling caused by certain venous diseases. It involves pushing fluids from the foot, leg, and thigh into the venous system by using a multi-chambered compression device.

New treatments for varicose veins

Recent press reports have generated considerable
interest in new techniques for treating varicose
veins, claiming major advantages over conven­
tional surgery. Their main aim is to reduce operative
trauma and bruising associated with stripping, leading
to quicker postoperative recovery. All these methods
depend on the use of duplex ultrasound scanning
during surgery to monitor obliteration of the vein
lumen. Evidence about these new techniques is limited
to case series and registry data, largely in private practice
settings. Many vascular surgeons have therefore
regarded the claims for their success with some
scepticism, especially when press reports have portrayed
conventional surgery in a falsely unfavourable light. The
new methods may well offer some advantages, but they
need further stringent evaluation.
Surgical stripping of the long saphenous vein is by
far the commonest form of treatment for varicose veins,
with more than 60 000 operations each year in England
alone. The reasons for treatment range from complica­
tions like bleeding or ulceration to the much commoner
complaints of discomfort or unsightliness. Indications
for specialist referral are the subject of recent advice
from the National Institute for Clinical Excellence.1 Sup­
port hosiery can control symptoms but many patients
dislike support stockings or tights; while injection
sclerotherapy is inappropriate for most symptomatic
varicose veins, because recurrence is common if there
are incompetent valves in the long saphenous vein.2 3
The established treatment for long saphenous vari­
cose veins is surgery,2 3 in the form of saphenofemoral
ligation and stripping, which involves an incision,
2­5 cm long, in the groin and a short incision, less than
1 cm, near the knee, with tiny incisions, 2­5mmlong, to
remove the varicosities (phlebectomies). Various
techniques are in use for stripping the long saphenous
vein, aimed at reducing the incidence of postoperative
bruising, and some surgeons ligate veins or use a tour­
niquet when doing phlebectomies. Compression
bandaging is standard practice, and many patients suf­
fer little bruising or discomfort. Some, however,
develop extensive bruising, lumpy haematoma, and
pain, especially in the context of large varicose veins,
obesity, and heparin prophylaxis. In the long term at
least one third of patients develop further varicose
veins, in either the treated or the untreated leg.
Three new methods have been promoted to close
off the long saphenous vein under ultrasound control,
avoiding a groin incision and gaining access to the vein
by a small incision or puncture near the knee. Radio­
frequency ablation4–6 involves use of a radiofrequency
probe to obliterate the vein by controlled thermal
injury. The probe is pulled slowly down the vein from
the groin with simultaneous application of pressure to
close off the lumen. The long saphenous vein can also
be obliterated using a laser probe.7 8 These techniques
can sometimes be used for veins other than the long
saphenous vein,5 but varicosities are usually removed
by phlebectomies in the traditional way.
A third new method (“varicose veins cured by a sin­
gle injection”) is a novel application of sclerotherapy, in
which sclerosant is mixed forcibly with air to produce a
foam that spreads rapidly and widely through the veins
after injection.9 Ultrasound monitoring prevents spread
of the foam into the femoral vein, although it seems that
the passage of small amounts into the deep veins is sel­
dom harmful. Sclerosant foam also fills many of the
varicose tributaries, so a single injection with subsequent
compression can obliterate these as well.
Proponents of all these techniques claim several
advantages over conventional surgery—in particular
reduced bruising and discomfort, with quicker return
to normal activity. Many surgeons undertake these
procedures under local anaesthesia, but general anaes­
thetic is necessary if there are extensive varicosities.

HYDERABAD: Five centres will be set up to issue tokens in advance for the Bathini fish medicine to be administered on Mrigasira Karthi day, said Hyderabad District Collector R Chandravadhan.He chaired a meeting held on Tuesday to discuss preparations for administering the medicine on June 8 and 9 in the city.The Collector instructed the officials to arrange proper lighting at the venue as the Mrigasira muhurtham was at 9.10 p.m. on June 8. He also advised them to be prepared for medical emergencies in view of the prevailing high temperatures in the city as they were likely to continue for some more time.Senior officials from the departments of fisheries, transport, water works, police and fire gave an update on the arrangements made by them.The transport department will arrange special buses, including one bus from the airport, to transport people to the Exhibition Grounds, the venue of the programme. The officials are also holding discussions with auto drivers to carry passengers to the grounds at nominal rates.The fisheries department plans to set up 33 stalls to supply fishlings required for the medicine. NGOs and voluntary organisations will take care of food arrangements at the venue. They will supply food specified for the fish medicine.Biotronics, the company behind the Sudarshan token scheme in Tirupati, will handle the advance token system. The firm will issue tokens from June 3 at special centres in the exhibition grounds and the Mandal Revenue Offices (MROs) in Nampally, Musheerabad, Secunderabad and Bandlaguda free of cost.The city administration and various departments will meet again this week to finalise an action plan for the event

Chinese Herbal Medicine for Psoriasis

Psoriasis is a relatively common skin disease, occurring in about 2% of the population. Onset is usually between 10 and 30 years of age, and tends to run in families. In its most common form. it presents itself with thickened patches of inflamed, red skin, covered with waxy silvery scales which tend to occur on elbows, knees, scalp, trunk, and back. This is known as plaque psoriasis. It is considered incurable by allopathic medicine, although acute flare-ups can subside, while severity and suffering vary considerably.Modern medicine treats psoriasis with ultraviolet light, topical steroid creams, tar creams, and even anti-cancer drugs such as methotrexate. While these treatments may help in the short term, they can cause rebound reactions which are more severe than the initial attack, or in the case of topical steroids, cause immediate worsening if the psoriasis is currently very active.On the other hand, Chinese herbal medicine can treat this illness completely in 60% of sufferers, and partially in the remaining 40%, over a period of several weeks.1 By the third to seventh week of treatment, there should be unmistakable reduction in symptoms, motivating the patient to persevere with the treatment until all plaques have gone.The main TCM diagnostic pattern in psoriasis is “Heat in the Blood,” and it is the pivot around which other secondary pathologies revolve. The Heat is clearly reflected in the redness of the skin lesions, as well as the often red tongue of the patient. Western medicine has identified that in psoriasis, skin cells are produced 10 times faster than normal, and this over-activity is another manifestation of excess “heat.” It is also noted that the guttate psoriasis subtype often occurs after a sore throat, which is localized invasion of Heat and Fire Poison. Unresolved, it can ‘spread’ its Heat to the skin. Unlike eczema and acne, psoriasis does not feature much Dampness in its presentation. This is shown in the lack of moisture, i.e. no weeping, crusting, or exudation. In fact, even where there are small, fluid filled pustules in pustular psoriasis, their contents are sterile.Having determined then that Heat in the Blood is the main pathology in psoriasis, the TCM practitioner must assess what co-factors are present, by looking at the visual characteristics of each patient’s skin. This step is important, and part of the reason why no generalized “psoriasis formula” can be very effective. Each patient’s presentation has to be addressed specifically.If the patient has a severe outbreak of new lesions, especially on the nails or palms and soles, recurring sore throat, and yellowish scales — in other words, if the psoriasis is very widespread and intense — it indicates Fire Poison is also present. Treatment will aim to cool the Blood and clear Fire Poison, with herbs such as Rx. Rhemannia Sheng Di Huang and Rx. Isatidis Ban Lan Gen.If plaques are covered in large amounts of scales and there is itching, which is not that common in psoriasis, this indicates Wind, and herbs like Rx. Ledebourielae Fang Feng must be added to the formula.If plaques are more purple than red, and the psoriasis is fairly inactive, Blood Stasis will be suspected. Purple, as we all know from observing bruising, is a clear indication of impaired blood circulation. This means Heat is bound with Stasis in a stubborn deadlock, manifested in this subtype’s poor response to many treatments. It can, however, respond well when herbs like Fl. Carthami Hong Hua are used in the prescription. There is a visual confirmation that the Stasis is improving in these cases: the lesions tend to enlarge slightly as they begin to heal, much as we might enlarge a knot’s surface area as we untangle it.TCM treatment for psoriasis works best in the form of herbal decoctions. This means that a formula composed of eight to 12 herbs is boiled together for about half an hour, and two cups of this liquid are drunk daily for the duration of the treatment. Lifestyle recommendations exclude anything that may add to Blood Heat, i.e. spicy foods, great mental and emotional stress, and skin damage such as tattoos and hard contact sports. 1. Ayurvedic treatment for PSORIASIS

Psoriasis is a non-infectious, inflammatory disease of the skin. It is characterized by well-defined erythematous (reddish) plaques with large adherent silvery scales. The main abnormality in psoriasis is an increased proliferation of the skin layers due to excessive division of the cell in the basal layers of the skin. Severe itching may associate with the plaques. Dryness of the skin and silvery scaling are characteristics of this condition.
1% to 3% of the most population has Psoriasis. It is very common among Europeans and North Americans. It is often seen between 15 to 40 yrs of age.
The exact reason behind Psoriasis is unknown. There is frequently a genetic predisposition. A child with one affected parent has a 15% chance of developing the disease and this rises to 50% if both the parents are affected. Streptococcal infection of the upper respiratory tract may exacerbate the condition. Deprivation of sunlight influences this condition and mental stress plays an important role in aggravating the condition. Certain drugs like chloroquine, chlorpropamide, lithium and practaol may aggravate this condition.
The peculiar lesions will be raised and red with dry, silvery-white scaling. Size may vary from a few millimeters to several centimeters in diameter. Common sites are scalp elbows knees and lower back. Pitting of the nails is very common. Involvements of palm is not uncommon. Many may develop sero-negative inflammatory arthritis, which is characterized by multiple involvements of joints and pitting on nails.
Ayurvedic view: Many basic Ayurvedic texts have explained skin diseases, which resembles Psoriasis. According these references, Psoriasis can be considered as the vitiation of Vata and Kapha. (Two of the basic energies/humors, which maintains our body equilibrium) Accumulation of low potency poisons (Dooshi vishas), are the basic pathological changes taking place in the system. Irregular food habits, consumption of foodstuffs that cannot be eaten together (Eg: diary products with fish), excessive intake of yogurt, black gram, seafood, sour and salted items etc can activate the pathogenesis. Alcohol and tobacco consumption will act as a catalyst here. Ayurveda do emphasize on the effect of stress in the pathogenesis of Psoriasis.
Treatment: Coming to the treatment aspects of Psoriasis according to Ayurveda, it aims at the detoxification of the body or elimination of toxins from the body fluids. It is achieved by "Panchakarma" treatments.
Current schedule, comprising various treatment aspects of Ayurveda is aiming at a better control of Psoriasis. This includes internal and external medications purely of plant origin. Rough out line of this schedule is as follows. (i) Consumption of medicated ghee, considering the Dosha type of the Psoriasis and the person, for 5 to 7 days. (ii) Detoxifying, by inducing vomiting (Vamana) and purgation (Virechana). This is followed by medicated buttermilk dripping overhead (Dahara) and pasting the entire body with a paste of medicines and mud. (iii) Basthies (medicated enemas) are being done for 8 days and internal medicines like herbal concoctions; medicated ghee has to be consumed for at least 90 days.
A strict diet regime has to be followed during the entire treatment period and it is better to continue as vegetarian. Yogurt, black gram, chilies and salted thing have to be restricted. It is better to avoid refrigerated/chilled foods. Psoriatic must keep a state of mental calmness.

Asthma treatment with 'fish medicine'

The fish medicine for chronic asthma patients, a traditional treatment known only to Bathini Gowd brothers of Hyderabad, will be administered free of cost for 24 hrs in the hyderabad city city in india.
The "miracle" asthma fish medicine of the Goud family is again being dispensed in the Andhra city of Hyderabad. This 153 year old tradition continues to draw sufferers from all over as it's believed to be an actual cure for the ailment. Apparently you swallow a live fish, for three days in a row and then stay on some special diet for 45 days. "Post-mortem", the patient is supposedly cured of asthma. Couldn't find any scientific or clinical studies so some skepticism may be in order, but placebo effects can go a long way.
